Conditions for Registering with SABO

1. You are not an employee as a referee of SABO or of any recreation program - you are an independent contractor.  SABO is merely obtaining basketball games to be assigned.  You are not covered by workman's compensation or any other insurance policy held or paid for by SABO or any recreation program.

2.  You are expected to keep a record of your games to use for verification purposes.  For each game you referee you will be paid a game fee according to the league and age of the players minus a seven percent (7%) booking fee.  Along with your check you will receive a list of all the games you called in each particular league.  If there is a discrepancy, let the booking agent know as quickly as possible.

3.  Because you are not an employee as a referee of SABO or of any recreation program, no withholdings will be deducted from your fee.  However the member recreation programs may report the fee on a Form 1099 or other appropriate Internal Revenue Service Form, so you may be earning reportable income.

4.  You agree that you will not hold SABO or its agent or any recreation program liable in the event of an injury or loss.  You are not covered by any insurance of SABO or of any recreation program, nor are you covered by Workman's Compensation.

5. You agree to abide by all SABO rules and regulations, and specifically to all terms listed above.

6. You understand that registering with SABO does not guarantee assignment of any games.

7. You will conduct yourself in a professional manner at all times.

8. You will will arrive on time for all game assignments.

9. You understand that you am not covered by any insurance of SABO or of any recreation program, and that you are not covered by Workman's Compensation.

If you accept these conditions, scroll down and print out the "Acceptance of Conditions & Assignments" and the "IRS W9" Form. Sign and turn these forms in to SABO for consideration of assignments.
